The health screening in Chicago Public Schools is an important step in reducing the likelihood of COVID-19 spreading. If you are a member of a certain group of students, staff, or visitors visiting the CPS School or any of the group’s buildings, you must pass the temperature check. If you plan to stay in the building for more than 10 minutes, you must pass this test.
The mobile phone must start the screening procedure by scanning the QR code or going directly to https://www.cps.edu/healthscreener.
How to Complete the Online Health Screening Process
To begin, go to https://www.cps.edu/healthscreener to see the official online health screening page.
The page can also be accessed via the URL https://www.cps.edu/healthscreener, and you can also access it by scanning the QR code with your phone’s camera.

- You will be able to enter the building after successfully completing the Screening.
- You must do the temperature check after completing the CPS health screener for students process and passing the screening.
- Anyone with a temperature of fewer than 100.4 degrees Fahrenheit will be eligible to pass.
- You will be sent home if you have a higher temperature (which signals a fever) or any other symptoms that indicate COVID-19.
- This health screening cannot be completed ahead of time; it must be completed on the spot.

What if I Didn’t Pass the Cps Health Screener?
If you fail the health screening, you are not permitted to enter the building and must leave immediately. You must understand that the entire screening process is in place to protect pupils and the surrounding area. They can’t let the sick or anyone who has come into contact with a Corona-infected individual into the building. The Cps Health Screener Has Changed — a New Process Has Been Introduced

Instructions must be read by all parents. The “AT-HOME HEALTH QUESTIONNAIRE” for families has taken the place of the Health Screener.
Families of the Holdens,
CPS has introduced adjustments to the Health Screener procedure that will make it much easier for families to get their children to school. The Health Questionnaire and Attestation form will take the role of the Health Screener. Please read the revised method below for more information so you know what to do before bringing your child to school each morning. This practice is essential for everyone’s safety.
Daily Health Questionnaire for At-Home Use:
Staff and children will use the CPS health screening questions to complete a daily health checklist at home before coming to school. There will be no electronic submission of the questionnaire. (See the attachment for a link to the staff screener.)
Parents and guardians will be needed to fill out an application.
a quarterly attestation form in which they agree to conduct daily health screenings on their child before sending them to school Parents and guardians will be asked to complete the following attestation: Agree to check their child at home each day before school for suspected symptoms or COVID-19 exposure.

Confirm that if they respond “yes” to any of the CPS health screening questions, they will not send their child to school. If their child will be absent from in-person class due to illness or exposure to a confirmed COVID case, parents should notify their child’s school. *For the first quarter, we need all families to sign and return the attestation forms to the school by September 2, 2021.
